New Patient Information
We want to provide you with excellent service! To help you prepare for your first visit, we recommend you print, complete, and bring these forms with you to your first visit.
- Be sure to arrive 15 minutes prior to your appointment
- Valid photo identification card Driver licensees passport
- Insurance card
- Be sure to bring all paperwork from referring physician:
- All surgical pathology reports pertaining to diagnosis
- Latest lab work: especially blood counts
- Any previous films and reports: PET scans, CT scans, MRI, X-ray, Mammogram
- Any previous chemotherapy/radiation treatment notes
- Physician’s progress notes
- We will do our best to request these items, but if you have them ready for us, this will greatly expedite your care.
- Co-pays are to be paid at the time of your appointment
